Financial Policy

Thank you for choosing Coleen M. Demas D.D.S., LLC as your dental provider.  Payment of your bill is considered part of your treatment and understanding of, and compliance with, our financial policies and procedures are important.  We realize that every person’s financial situation is different.  For this reason, we provide a variety of payment options to help you receive the dental care you need and deserve that allows you to enjoy a healthy, beautiful smile with respect to your budget.  Dental treatment is an excellent investment in an individual’s medical and psychological care.  We are always available to answer your questions or assist in any way we can.

Dental Insurance Policies & Procedures

We accept dental insurance and will gladly file insurance claims for you as a courtesy.  Please verify with your insurance company that you can come to our office and specifics about your coverage.  All insurance claims that go unpaid for more than 60 days are the sole responsibility of the insured.  Please be aware that we will assist you in filing claims with your insurance company, however, we are not responsible for any denied claims and they are the responsibility of the insured.  Your estimated out-of-pocket costs are due at the time services are rendered.

Patients without dental insurance are expected to pay at the time services are rendered.  There is also a $30.00 fee for for checks returned for insufficient funds.

Optional Payment Terms

  • Major Service – Two Payment Option: We offer a two-payment option for Crown, Bridge, and Debture treatments.  We ask that you pay one-half of your co-payment at the first appointment and the second half at the seat date appointment.  We accept cash, checks, debit cards, and all major credit cards.
  • Credit Care Payment Option: We allow (with established payment history with our office), a Credit Card Payment Option, which allows you to make four equal installment payments by credit card.  On the 15th of each month our office personnel will charge those payments to your credit card in the appropriate amount.
  • Term Loan: By arrangement with Care Credit, we offer patients, upon approval, an interest-free term loan (6 to 12 months) with no down payment, no annual fee, and no prepayment penalty.  Please visit www.carecredit.com for further information and instant application approval.

Last Minute Cancellation or Forgotten Appointment Policy

There will be a $50.00 per scheduled hour administrative charge for a forgotten appointment without notification or a last minute cancellation (i.e. without 48 hour notice).  Numerous missed appointments may be reason for patient dismissal.

Parent / Guardian Billing Responsibility

The parent or guardian who brings in a child for services and approves treatment is responsible for all fees.  We do not bill a separate party in the case of divorce or separation even when a divorce doctrine may state differently.  Reimbursement must be made between divorced parents.

Also note that we request that parents wait in the waiting room while treatment is being performed on their children.  We appreciate your cooperation in this manner.
